The case management department is comprised of a staff of qualified case managers who individually deal with TACT clients, helping them cope with physical, emotional and financial difficulties. Case managers can offer information and help in gaining access to:

Public aide or Social Security programs

Support groups for HIV-positive and their friends/family

Rent subsidies, and financial assistance for outstanding bills for those that qualify

In-home services for disabled individuals with HIV who need help with meals, laundry, shopping, bathing, etc.

Medical services and home health services

Food and nutrition programs
-
Nutritional supplements
-
Legal services such as Powers Of Attorney for health care, property, living wills and referral for Legal Services

Housing
-
Substance abuse treatment programs

Mental Health programs

Dental services

Rehabilitative services

Employment services

Transportation services
Emergency client assistance and rent subsidies are available only to individuals who are receiving case management services with TACT and meet the current qualifications.

Heath Insurance Card
Case Managers will make an assessment of client's needs and will review all HIV-related health conditions. Jointly with the client, a Case Manager will also develop a service plan to meet the clients needs.
